"Chan" is the one you use when you're familiar with someone, usually with girls or younger boys. "San" is the one you use when you're with someone you dont' know well or respect.

Even if Shouto suspects it, I think he'd be really happy if you told him directly. Sometimes what's obvious to us isn't as obvious to everyone else. I thought I was being really obvious with my feelings before, too, but it turns out he had no idea until we started dating.
